Conditioning

In order to participate:

Step 1

  • Complete a sports physical with La Clinica or your primary care provider. 
  • If it is your first time at La Clinica, you will need to complete a registration form.

  • The physical consists of the PPE History AND MedCard, linked below or available for pickup at CCPA during distribution hours.

Note - you will need to upload a picture or pdf of your physical in step 2.

Step 2

  • After your physical is done and you have the PPE History and MedCard completed, complete the OAL Registration Form. 
  • Make sure to upload your physical (all pages).  If you have trouble uploading, you may also email your forms directly to Ms. Boyle.

Step 3

  • Check your OUSD email for confirmation. Message Ms. Boyle on Jupiter with any questions.
